Grant Application

Partnering for Health Equity

A new grant opportunity from Cone Health Foundation

Why We’re Doing It

Everyone should have the opportunity for good health.

Access to high-quality medical care is critical, but research shows up to 80 percent of a person’s health outcomes are influenced by factors such as access to healthy food, reliable transportation, and financial stability.

As Cone Health Foundation (CHF) sets strategic priorities aligned with our mission and vision, we know we must move beyond simply addressing access to health care. We hope to influence the social drivers of health with racial equity at the center — this Request for Proposals is a step in that direction.

Cone Health Foundation values access to health care, access to healthy food, safe and healthy housing, economic mobility, and education

Opportunity At A Glance

We want to partner with organizations in Greensboro working diligently to address systemic health inequities by impacting the social and environmental factors that drive health equity in the following priority areas: healthy food, education, safe and healthy housing, and economic mobility.

As we learn more about those factors, CHF is offering one-time, non-renewable grant support of up to $10,000 that can be used for:

  • Capacity building
  • Community building
  • General operating expenses.
  • Program support or development

CHF will distribute up to $250,000 through this grant process.

Grant applications are due Feb. 7, 2025 by 5 pm EST. Grant announcements will be made by the end of March.

Who Can Apply

While we understand the importance of stable, multi-year funding, this opportunity is not able to support repeat funding requests. One-time funding requests up to $10,000 will allow CHF to support a broad spectrum of organizations and fiscally sponsored projects that:

  • Have been overlooked by — or have had limited access to — funders. “Newer” and “smaller” organizations (with operating budgets less than $500,000) are encouraged to apply. (Organizations who have received multi-year funding from Cone Health Foundation and former Nurturing Equity grant recipients are not eligible to apply.)
  • Are founded, led, and governed by people who reflect the communities they serve. We believe the most equitable and effective approaches come from organizations whose decision-makers reflect the people they serve.
  • Represent place- and/or identity-based communities most adversely impacted by inequitable and discriminatory policies. CHF seeks to fund work by and for Black, Indigenous, and other communities of color; queer, transgender, and gender-nonconforming people; people with disabilities; immigrants; refugeees, people living with low to moderate incomes; people who are involved with the justice system; and especially those who hold several of these identities. The service/funding area is limited to Greensboro.
  • Are nonprofit organizations and grassroots groups. Organizations without a 501c3 status but who have a fiscal sponsor can apply. The service/funding area is limited to Greensboro.
  • Are pursuing community-powered work that centers racial justice and equity. Those efforts might include: mobilizing people and organizations, developing leaders, fostering connections, leading anti-racism efforts, and the many other ways historically marginalized communities express power.

Below are requests that will not be considered:

  • Capital campaign requests
  • Debt retirement/deficit funding
  • Duplication of existing programs or services by new organizations
  • Endowment funds
  • Previously incurred costs
  • Projects that are typically the funding responsibility of federal, state, or local government
  • Publication of books
  • Requests from CHF grantees with current grant agreements in place
  • Requests from former Nurturing Equity grant recipients
  • Sponsorships and fundraisers

Restrictions: Projects that promote specific political or religious views or engage in political or lobbying activities are also ineligible.

Please Note
Funded grant partners will be required to submit a year-end financial and progress report.
